We are located in the Thunderbird Commons complex Northwest corner of Thunderbird and 67th Avenue. Our address is 6790 W. Thunderbird Rd. Peoria AZ 85381. View Map and Directions.
2. What are your hours of operation?

Routine business hours are 6 a.m. to 5 p.m. Monday – Friday.
3. Where do I check in?

You will want to sign-in at the front desk receptionist – you will be registered in the order of your scheduled procedure.
4. What should I wear?

Dress comfortable. Loose fitting apparel and slip on shoes.
5. What should I bring with me?

It is important that you bring your Insurance Card, Proof of Identification (Drivers License) preferred, and Co-pay. You may have been instructed by our Pre-operative Nurse to bring a list of current medications, previous surgeries and X-rays (if applicable).
Please do not bring any valuable jewelry to our center.
6. Can I drive myself home?

You must have an adult driver to drive you home and stay with you for the first 24 hours if you have received any sedation for your procedure. If you have had a straight local anesthesia with no sedation you may drive yourself home. Please call us if you in advance if you have any question regarding this matter.
7. My child is having surgery; do I have to stay at the surgery center during the procedure or can I leave and return once the surgery is over?

If you are the legal guardian or parents of the child (one parent) must remain in the facility at all times.
8. Can I wear my jewelry or body piercing for my procedure?

We suggest you leave all valuables at home. If your body piercing can be removed please remove prior to your procedure. You will be required to remove any body piercing at the surgical site or if it may interfere with your airway.
9. Can I eat or drink before my procedure?

Your physician’s office may have already provided you with instructions on what and when you should stop eating or drinking. A nurse from Pinnacle Surgery Center will also contact you before your procedure to discuss when you should stop eating and drinking before your procedure. If you have any questions call the facility and request to speak to a Pre-Operative Nurse. for Instructions.
